How to Apply

1. Apply to ÃÛÌÒAPP online. You should receive an email with your student ID number within about 24 hours.  
2. . Make sure to indicate that you are currently in high school and in 12th grade.  Attach the following documents to your application:
   • Your most recent IEP/504 Plan (if applicable)
   • Your most recent Triennial/Multi-disciplinary Assessment Report (if applicable)
   • Your unofficial high school transcript
   • Any other disability verification documents such as information from your doctor/medical provider, etc. (if applicable)
3. Keep an eye on your emails, voicemails, and texts, as our office will be contacting you over the next few months to schedule individual appointments with our counselors/specialists.
